Structural genomic alterations, non-coding variants and regulatory networks. The revolution of future in cancer therapy.

Abstract
In 2012, the ENCODE PROJECT integrating NGS technologies and computational systems biology methods toward understanding human genome functionality has revolutionized biomedical research. Although less than 10% of genome and transcriptome in health has been understood, the new knowledge of non-coding genome functionality and dynamics of transcriptional networks represent the formation of future Medicine.
This perspective article analyzes the findings of large-scale Whole Exome Sequencing (WES), Whole Genome Sequencing (WGS), and RNA-sequencing (RNA-seq) studies of conventional single biopsy analysis as well as intratumor heterogeneity repeated cell-free DNA – NGS (cfDNA – NGS) for revealing dynamic emergence of resistant subclones.
Moreover, data published non-coding DNA are discussed also with future perspective for understanding regulatory networks controlling, gene activation or sustainable multiple interactive genes and delineated in figure 1.
Challenges in understanding and predicting the outcome of highly complex biological systems as a complete system and their components interactions deregulations structural genome and transcriptome challenges are also discussed.
Finally, the current unresolved problems of disrupting regulatory networks by comprehensive targeting of both structural whole genome variation and agents targeting regulatory transcriptional bio-circuits are analyzed.
