COMMENTARY

First randomized evidence for the superiority of extended over limited lymphadenectomy in gastric cancer: A never end debate.

As this article has no abstract we provide the first paragraph.

For the first time, evidence is provided by a randomized controlled trial that extensive (D3) vs. limited (D1) surgery significantly improves overall survival. But However, the difference in recurrence-free survival between the two groups was not statistically significant. This paradox and a number of study weakness arises the following questions:

  • Are the results of this trial conclusive?
  • Is there an end in the long-term debate about the benefit of extended lymphadenectomy?
  • Do we need new randomized trials?
  • Until then what extent of lymphadenectomy can be considered as optimal?
